The consent form must include the names of the trustees (including the designated chairman and secretary), as well as their goals and objectives. ● Following approval, two national newspapers publish the article, and there is a 28-day waiting period before filing. ● Uploading the meeting minutes, the trustees' identity, and the chairman and secretary's signatures completes the filing process.
